Job Description Summary:
The primary responsibility of this role is to provide essential care and support to patients receiving home health or hospice services while reporting to the Clinical Supervisor(s) and the Registered Nurse in charge of the patient's case. The ideal candidate will demonstrate competence in delivering personalized care while promoting dignity, comfort, and independence for patients in their homes.
Job Duties:
- Provide personal care assistance to patients as directed by the supervising registered nurse or healthcare professional.
- Assist patients with activities of daily living, including bathing, dressing, grooming, and toileting.
- Help patients with mobility and transferring as needed, ensuring safety and comfort at all times.
- Administer prescribed medications and assist with medication reminders under the supervision of a licensed healthcare professional.
- Monitor patient vital signs and report any significant changes or concerns to the supervising nurse or healthcare team.
- Maintain a clean and organized environment for the patient, including light housekeeping tasks such as tidying up living spaces and laundering linens.
- Provide companionship and emotional support to patients and their families, fostering a caring and supportive environment.
- Assist in meal preparation and feeding as necessary, adhering to any dietary restrictions or preferences.
- Document care provided accurately and timely in accordance with agency policies and procedures.
- Participate in interdisciplinary team meetings and communicate effectively with other healthcare professionals involved in the patient's care.
- A dhere to all applicable regulations, guidelines, and standards of practice governing home health and hospice care.
- Demonstrates commitment, professional growth and competency by completing 16 hours of education per year.
- Home Health Aides must demonstrate proficiency in procedures outlined in their job description. Effective communication with healthcare professionals, patients, and families is imperative.
- They should have reliable transportation to make home visits and assess home safety. Skills in problem-solving, considering patients' backgrounds, finances, and resources, are essential for delivering optimal care.
