We are seeking a motivated Intake Coordinator LVN to join our multidisciplinary hospice team. As an Intake Coordinator, you will play a crucial role in the hospice setting by facilitating the intake process for patients and ensuring a seamless transition into care. Your organizational skills and attention to detail will be essential in this position. LVN or RN License required. Hospice or intake experience is required.

Spanish strongly preferred.

• Collect and record patient information accurately, coordinate patient appointments and referrals
• Verify insurance coverage and benefits
• Communicate effectively with doctors, patients, healthcare providers, and insurance companies
• Confirm billing risk and obtain prior authorizations as needed, follow up to bring outstanding auths to resolution
• Build and maintain relationships with community partners
• Demonstrate bilingual proficiency to assist Spanish-speaking patients

• Previous experience in hospice or palliative care required - must have prior hospice experience to apply
• Strong organizational skills and attention to detail
• Ability to communicate effectively within a multidisciplinary care team
