JOB REQUIREMENTS:
Provides case management through CLTS Support and Service Coordination to children who are diagnosed with physical, emotional, or cognitive disabilities. The goal of the CLTS Waiver Program is to support children with substantial needs, as well as their parents/guardians, by delivering services to promote the goal of every child living their best possible life now and into the future. This position may be eligible for a hybrid schedule to telework up to 3 days a week after successful completion of an initial employment period.

Responsibilities- Support and Service Coordination assists a child or youth and his or her family to achieve an inclusive, independent, and self-empowered life.
- Use knowledge of available programs, resources, and services to create an individualized service plan that best supports the child and family in pursuit of their desired outcomes.
- Connect families to supports throughout the community and promote family members self-determination and involvement in all facets of community life.
- Conduct monthly collateral contacts and direct contact with family every three months; assist to establish and maintain all aspects of program functional and financial eligibility.
- Provide participant and family education, support, and advocacy regarding the disability; assess and periodically reassess the child s or youth s health, safety, and functional capacity.
- Report incidents that create significant risk or services harm to the WI Department of Health Services; assure application of client rights, grievance and dispute resolution processes and procedures.
- Complete authorizations for purchase of service and obtain supervisory approval in line with service plans; monitor contracted services to ensure high quality and appropriate extent of services.
- Interface with healthcare providers/insurers/Medicaid/Social Security to coordinate services; assist families with transition to adulthood and connect them to resources as needed.
- Identify the need for and assist in planning and development of new service resources; comply with applicable county, state and federal guidelines related to service provision.
- Complete and maintain accurate and timely documentation; facilitate collaboration and maintain positive relationships with participants, informal support systems, co-workers, health professionals, courts, legal representatives, contracted providers and the public.
- Attend and contribute to staff meetings and overall unit performance; function as part of treatment teams/multi-disciplinary teams to ensure continuity of care.
- Knowledge of case management principles and methods, family dynamics, human development, and behavior.
- Knowledge of dynamics involved in community-based care for persons with long-term care needs.
- Knowledge of social work assessment principles and techniques; knowledge of client rights, grievance and dispute resolution processes and procedures.
- Knowledge of state and federal laws, codes, procedures, and policies pertaining to Medicaid and Wisconsin Long-Term Care program delivery.
- Knowledge of community resources and needs and dynamics of those with all types of disabilities.
- Ability to establish priorities for service coordination and to consider alternative treatment plans.
- Ability to work independently; ability to receive and provide feedback; be decisive and make sound decisions under pressure of urgent situations.
- Ability to plan and organize work to meet deadlines and make effective use of time; establish and maintain positive relationships with consumers and their family members, staff, contracted providers, and the public.
- Ability to communicate effectively, both orally and in writing.
- Ability to work the required hours of the position, which is 37.5 hours weekly, with core hours being 8:00am-4:30pm.
