Behavioral Health Care Manager

The LPN Member Health Assessor will work in remote and field settings supporting our Medicaid SMI population in the state of South Carolina. This position is for a (LPN) Licensed Practical Nurse who lives in South Carolina and must be licensed for the state of South Carolina. The LPN Member Health Assessor will partner with Behavioral Health Case Managers and meet with Medicaid members to complete/sign care plans and complete social determinant assessments.

Preferred candidates will have experience working with the behavioral health population. Excellent computer skills and attention to detail are important to multitask between systems, talk with members on the phone, and enter accurate contact notes. This is a fast-paced position and productivity is important.

• Work in remote and field settings supporting the Medicaid SMI population in South Carolina, partnering with Behavioral Health Case Managers and meeting with Medicaid members to complete/sign care plans and complete social determinant assessments.
• Travel (up to 60%) in the field to meet member visits in the surrounding areas (Columbia, Aiken, Lexington).
• Maintain accurate contact notes and multitask between systems while communicating with members by phone.
• Work from a home office with internet connectivity of high speed.

• Schedule:
  
  Monday through Friday 8:00 AM to 5:00 PM EST; no weekends or holidays.
• Home office with high-speed internet connectivity required.

• (LPN) Licensed Practical Nurse licensed in South Carolina.
• Must live in South Carolina and be licensed for the state of South Carolina.
• Experience with the Behavioral health population is preferred.
• Excellent computer skills and attention to detail; ability to multitask between systems, talk with members on the phone, and enter accurate contact notes.
