Key Responsibilities
:

  • Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
  • Perform independently under stress and in emergency situations.
  • Drive between locations, navigating to each location using GPS device or other tools.
  • Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility, and other personnel involved with patient care.
  • Assess health status by obtaining relevant health and medical history; perform a physical exam and identify significant health and medical risk factors.
  • Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs, developing a diagnosis by analyzing and synthesizing collected data.
  • Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges; prescribe and monitor medications as allowed by prescriptive authority.
  • Develop a treatment plan in collaboration with the patient and significant others, ensuring quality patient care with multidisciplinary teamwork.
  • Refer patients to providers as appropriate for consultation or specialized health resources and treatment, ensuring continuity of care.
  • Supervise nurse practitioners and physician assistants per the collaborative agreement.

Required Qualifications
:

  • Doctor of Medicine or Osteopathy required.
  • Current unrestricted State license to practice medicine.
  • Prior supervisor/management experience.
  • Demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively.
  • Proven organizational development and project management skills.
  • Working knowledge of regulations and reimbursement in home health care with proven leadership abilities.
  • Ability to sit, stand, bend, lift, and move intermittently; lift at least 50 lbs.
  • Current and valid driver’s license with reliable transportation and valid automobile insurance coverage.

Desired Qualifications
:

  • Board certification in family practice, internal medicine, geriatric, or palliative care preferred.
  • Preferred 2 years experience in healthcare, with a combination of acute care, geriatric, and/or long‑term care practice.
  • Experience with geriatric patients preferred.

Location
:

Home Office: 770 Kenmoor Ave, Suite 100, Grand Rapids, MI 49546. Main service area:
Grand Rapids and surrounding; this position services patients in facilities and private home settings.

Hours
:

Office hours: 8:00 AM – 5:00 PM, Monday through Friday.

Pay Range
:

$150,000—$200,000 USD.
