A leading national healthcare organization is seeking a full-time, board-certified Palliative Care Physician to join its growing team. This physician will provide compassionate, patient-centered care to individuals with complex chronic conditions such as CKD/ESRD, COPD, and CHF.

• Deliver palliative care services to patients in the community through a combination of home visits, tele-health and APP supervision
• Manage symptoms affecting quality of life, coordinate referrals, educate care teams, and track clinical and financial outcomes

• Fellowship training and/or BC/BE in Hospice and Palliative Medicine
• Experience with all populations and payer mixes including high-need medicare, medicaid and commercial
• Comfort and skilled in care discussions, advanced care planning, symptom management, specifically complex medication panels, acute and behavioral health care and appreciation of social determinants
• Experience and willingness to supervise and work closely with advanced practice providers
• Licensure or ability to become licensed in Mississippi
• Must have reliable transportation and be willing to drive within the Jackson, MS area as needed
