We are seeking a collaborating board certified Family Medicine:
Sports Medicine Fellowship trained physician for our APP providers. This individual will be needed solely for licensing, credentialing and collaboration only as required by Texas law. This individual must be willing to move quickly for our staff to update CAQH profiles in preparation for credentialing submissions
- Education:MD/DO degree from an accredited medical school
- Licensure:Texas
- Certifications:Board certification in Sports Medicine, Physical Medicine & Rehabilitation, or similar
