• Serve as the primary care provider for assigned patients, managing preventive, acute, and chronic health needs.
• Perform comprehensive health assessments, order and interpret diagnostics, and guide personalized care plans.
• Collaborate with an interdisciplinary team of Nurse Practitioners, Physician Assistants, and Wellness Coordinators.
• Lead population health initiatives and quality improvement programs that enhance patient outcomes.
• Provide mentorship to mid-level providers and clinical support staff.
• Participate in regular provider meetings and professional development to advance clinical excellence.

• MD or DO with an active California medical license.
• Board Certified in Family Medicine, Internal Medicine, or Geriatrics.
• Experienced in senior or geriatric care (preferred).
• Skilled in EMR documentation and team-based collaboration.
• Compassionate, communicative, and driven by patient-centered outcomes.

• Monday–Friday schedule – no nights or weekends required.
