As a Primary Care Physician, you’ll serve as the central provider for your patient panel, delivering high‑quality, comprehensive care in home‑based and community settings, supported by an experienced interdisciplinary team.

• Providing preventive, acute, and chronic care for senior patients
• Conducting comprehensive health assessments and managing personalized care plans
• Ordering and interpreting labs and diagnostics
• Collaborating closely with Nurse Practitioners, Physician Assistants, and Wellness Coordinators
• Leading population health and quality improvement initiatives
• Mentoring and supporting advanced practice providers
• Participating in provider meetings and continuing clinical development

• MD or DO with an active California medical license
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Experience with geriatric or senior‑focused care (strongly preferred)
• Comfortable with EMR systems and team‑based care models
• Compassionate, communicative, and patient‑centered

• Monday–Friday schedule – no nights, weekends, or call
• Highly competitive compensation
  : $215,000–$350,000 annually, based on experience and productivity
