Family Practice opportunity at leading Health Care Facility in Lawrence, MA

Physician Provides comprehensive and continuing care to patients requiring routine and emergency treatment through the diagnosis of disease processes and through the provision of medical treatment of patients at community health centers.

Physician serves as a primary care provider to a panel of patients and is responsible for organizing and managing the care of primary care patients, including outpatient visits, communication with patients, inpatient care and referral for specialty care within the Patient Centered Care Model. Physician assesses health risks, promotes and provides disease prevention, and practices evidenced based medicine
