Overview
The Family Preservation Services Case Manager provides for the safety, stability and well-being of children and families by delivering and coordinating culturally competent case management services that are responsive to the unique needs of the family.
Essential Functions- Provide for the safety, stability and well-being of children and families during the referral period.
- Deliver culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models.
- Make decisions based on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model and DCCCA organizational policies.
- Meet DCF timelines to connect with the family within 24 hours of referral; facilitate an initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3–6 months of referral.
- Engage and work in partnership with families to complete assessments and develop case plan goals and objectives.
- Meet with the family in the home weekly, monitoring progress toward case plan goals; meet monthly with each child individually to assess child safety.
- Assess each child s safety with every family contact and report all suspected child abuse/neglect. Monitor family progress and service provision using assessment tools, and update the family case plan based on those assessments.
- Provide weekly home-based family skills training which may include nutrition, hygiene, household and time management, home and child safety, basic parenting, transportation, problem solving and communication to support a safe, nurturing environment for children in their care.
- Work closely with the Family Support Worker to provide services for the family and make decisions about child well-being.
- Prioritize identified problems, establish goals and objectives, utilize available resources, and reach decisions appropriate to the family.
- Complete and analyze initial and ongoing family assessments related to safety, stability and well-being (mental health, substance abuse, education, emotional, social and physical well-being, parenting and parent-child relationships).
- Use assessments to develop the case plan in partnership with the Family Support Worker/Family Mentor, child(ren) and family; make community referrals as necessary.
- Complete and submit timely court reports; appear and testify in court as required.
- Collect drug screens as necessary.
- Document case information, including court reports, emails and activities weekly.
- Respond promptly to family crisis situations, document incidents, and be available 24/7 to clients.
- Job may require the employee to regularly drive to various DCCCA locations and other business locations throughout the day or night as required.
- Promotes and practices teamwork in all activities; fosters and maintains professional relationships with families, community partners and peers.
- Participates in professional development that is relevant.
- Bachelor s degree in human services is required.
- Prior work in behavioral health or children/family services is required; personal experience with addictions treatment is preferred.
- Must possess strong verbal and written communication skills.
- Demonstrates flexibility and adaptability; able to shift styles to fit the needs of a wide range of cultures, people, and organizations.
- Solid commitment to the principles of support and advocacy for foster and adoptive families.
- Must possess a valid state Driver s License, in-force automobile liability insurance, and maintain a Motor Vehicle Record (MVR) that meets organization s insurability standard as defined by policy. Availability of reliable transportation is required for this position.
