Overview

Hope For Youth is seeking a Case Planner for our HEART program. Hope For Youth is a nonprofit organization specializing in child welfare services. For over 50 years, Hope For Youth strives to provide positive outcomes for children, adolescents, and families.

Hope For Youth's Intensive Preventive Services Program, funded by Nassau County Department of Social Services, is a home-based, time-limited, family-oriented, supportive and rehabilitative child welfare service. The focus of HEART is to avert a disruption of a family that could result in the imminent placement of a child in foster care, enable a child who has been placed in foster care to return to his/her family earlier, and reduce the likelihood that a child who has been discharged from foster care would return to such care.

Responsibilities
  • Function within Agency s policies and procedures as outlined in the Employee Manual.
  • Function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
  • Provide support to the clients and families—i.e., the knowledge, skills and tools necessary in order for them to maintain the child in their home.
  • Achieve successful outcomes within the guidelines outlined in NYS, County and HFY policy and procedure (shared).
  • Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations.
Time/Activity Outline
  • Maintaining regular contact with the youth and their families at least twice a week, one contact being in the home and implementing the various parts of the program model (including evenings, nights and weekends) for caseload.
  • Completing the initial CANS-NY and all subsequent CANS-NY, attending school meetings and court appearances with the families as necessary, and being responsible for discharge/transition planning.
  • Providing case management services, life coaching, parent skill training, and making all collateral contacts as necessary.
  • Regularly reviewing the service plan with the youth and family; meeting with supervisor to review case record.
  • Documentation and related paperwork.
  • Staff meetings, treatment team meetings and conferences.
  • Training and staff development.
  • Other related assignments as needed to be successful.
Minimum Qualifications
  • BSW (or Bachelor's in a related field) plus 2 years related casework experience or MSW.
  • Excellent written, verbal and computer skills.
  • Flexible schedule to accommodate clients and program.
  • Valid NYS Driver s License in good standing.
  • Physical Requirements:

    read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving own vehicle.
Position Details

This opportunity is being offered as a full-time position, 35 hours per week, non traditional.
