Description:

Oneida Health is seeking a Licensed Practical Nurse (LPN) to provide care for residents in a Rehabilitation and Extended Care Facility.
The LPN will work varying shifts and will collaborate with the Unit Manager, Registered Nurse, Physician, Certified Nursing Assistants, and Ward Clerks.
This role requires excellent communication skills, sound nursing judgment, and a compassionate approach to patient care.
The LPN will provide compassionate care and treatment to residents while maintaining a customer service attitude.
Collaboration with the Interdisciplinary Team (IDT) is essential, including Physicians, Registered Nurses, Unit Managers, Certified Nursing Assistants, and Ward Clerks.
The LPN will use sound nursing judgment to evaluate and observe changes in resident conditions.
Quick and clear communication with Registered Nurses and Physicians regarding changes in resident situations is crucial.
The LPN will delegate assignments and observe support staff to ensure appropriate care is provided according to treatment plans.
Responsibilities include ordering medications and supplies, preparing equipment, and assisting with wound care as needed.
Maintaining a clean and healthy environment for residents is important, including ensuring hygienic dining areas and personal hygiene spaces.
Scheduling resident appointments and notifying families about appointments and transportation is required.
Orders will be input into the computer system, notifying the pharmacy while maintaining accurate record-keeping.
The LPN will attend meetings as directed by the Unit Manager/Supervisor and participate in care meetings and discharge planning.
Compliance with policies and procedures related to controlled substances and documentation is expected.
Reporting building emergencies or concerns promptly and accurately is necessary.
Incidents regarding suspected abuse, neglect, mistreatment, and exploitation must be reported.

Education:

High School diploma or equivalent is required.
Completion of Licensed Practical Nurse course of study is required.

Physical Requirements:

Ability to lift objects weighing 45 pounds or more is necessary.
Skill in carrying heavy items weighing 45 pounds or more is needed.
Demonstrated ability to pull and push objects as required.
Capabilities to reach above shoulder level and perform tasks requiring hand-eye coordination are essential.
Proficiency in walking, standing, and sitting for extended periods during work shifts is important.
The ability to kneel and perform repeated bending motions as needed is required.
Capacity to operate motor vehicles is necessary.
