- Active, unencumbered LPN/LVN license in the state where the clinician will practice

- Six months of hands‑on nursing experience preferred but not required

The Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians' orders, while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition, reporting changes to supervisor or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in, implement, and update the nursing care plan.
- Take appropriate nursing action based on assessment to achieve expected outcomes.
- Recognize changes in patient needs and intervene to prevent or reduce risk.
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Apply infection control measures that protect both staff and patient according to OSHA standards.
- Educate patient and family regarding disease process, self‑care techniques, and prevention strategies.
- Maintain knowledge of nursing competencies through continuing education and internal training.
- Graduate of an accredited nursing school.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.
- South Carolina – One year of pediatric experience.
- California – One year of experience working under a current nursing license.
- Louisiana – One year of experience working as a licensed nurse.
- Continuing education as required by state.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Speak, write, read, and understand English.
- Travel as required.
- Lift 50 pounds.
- Reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
