• Help patients with needs over the weekend
• Conduct in-home assessments to determine eligibility for home health services
• Collaborate with patients, families, and interdisciplinary teams to create individualized care plans
• Coordinate with physicians and other healthcare providers to ensure continuity of care
• Provide patient education and support to promote self-management and independence
• Ensure compliance with regulatory requirements and quality standards

• Current RN licensure in the state of Nebraska
• Previous experience in home health or hospice care preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient-centered approach to care
