Qualifications- Valid Registered Nurse license in the state of Nebraska
- Excellent computer skills
- Excellent organizational and communication skills a must
- Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance

As a RN Case Manager, you will- Visit with patients in their homes or nursing facility setting, providing assessment, teaching and skilled care
- Work with and receive support of other professionals to problem solve and update the patient's plan of care
