. The position is available at the Methodist Physicians Clinic – Gretna, 11946 Standing Stone Drive, Gretna, NE. The work schedule is Monday‑Friday, 8 am–5 pm.
As a triage nurse, you will provide telephone triage service to enhance compliance with patients’ plans of care and ensure prompt identification of patients with high‑risk conditions. You will sort and prioritize patients through assessments, monitoring, recording and changes in their condition. Precision and accuracy in maintaining detailed assessment and reports is essential.
Responsibilities- Perform telephone triage to determine patient needs by recording and reporting patient condition to provider and advising patient based on provider instructions.
- Receive and return patient calls within 90 minutes based on medical priority 95% of the time.
- Accurately record and report patient’s condition according to department policy 98% of the time.
- Strictly adhere to practice protocols.
- Consult patient’s chart while talking to the patient.
- Return phone messages according to protocol at least 98% of the time.
- Provide test results according to protocol at least 98% of the time.
- At the direction of the provider provide patient and family education to ensure adequate knowledge/awareness and recommend appropriate community resources as needed.
- Educate/instruct patients according to provider direction 98% of the time.
- Document all lab, procedures, and appointments on tracking forms.
- Complete tracking forms and follow up on missed appointments and procedures.
- Assist Health Coaches by working patient lists and scheduling appointments as directed.
- Maintain work areas to ensure healthy environmental conditions by cleaning and organizing equipment and rooms.
- Handle prescription renewals/inquiries to facilitate patient care. Discuss questions with provider and relay information to pharmacy.
- Handle prescription refills according to department provider protocols 99% of the time.
- Accurately complete pharmacy requests 100% of the time.
- Provide appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the “Age Specific Criteria”.
- Complete nursing assessments.
- Use nursing judgment to develop plan of care.
- Determine severity of patient illness and intervene to treat while prioritizing patient calls.
- Document clear and concise notes describing phone call utilizing SBAR format.
- Communicate between provider and patient appropriately.
- Conduct patient and family education; evaluate and document in chart the patient’s understanding of all education.
- Coordinate continuation of care through referrals to specialists.
- Prepare exam rooms and equipment to facilitate treatment and examination of patients by setting up instruments/equipment and stocking supplies.
- Prepare patient for provider exam by reviewing medical records, weighing patient, checking vital signs, performing medication reconciliation and obtaining information relevant to patient care.
- Assist providers to facilitate patient treatment by retrieving tests and medical records and obtaining lab work as directed.
- Obtain provider requested tests, records, and lab work 95% of the time.
- Dispense medications at the direction of a provider to patients. Registered Nurse and Licensed Practical Nurse instruct patients and family members regarding education and treatment to ensure patients understand how to take medication correctly.
- Assist and/or perform phlebotomy with proper labeling and handling of specimens, accurately completing requisitions, and collecting specimens according to standard procedures/protocols and performing and documenting quality control activities.
- Perform above procedure according to clinic policy and procedures 95% of the time.
- Ensure accuracy of order entry and complete appropriate documentation without error at least 98% of the time.
- Graduate of an accredited LPN program required.
- Minimum 1 year clinical experience required.
