Position Summary

The Medical Case Manager will provide various levels of case management services to individuals living with HIV / AIDS including assessment, referral and support for participants of Ryan White grants. This position will assist patients enrolled in Ryan White services as well as assist with housing instability, food insecurity, mental illness and other issues through a referral network. This position will assist patients with Ryan White enrollment and renewals including State, Federal and community programs.

This individual will support the Ryan White team and clinical staff.

Required Qualifications
  • Required

    Education:

    Bachelor s degree or equivalent. If any degree major/training is required, please specify the type. Public Health, Healthcare Administration, Social Work, Human Services, or related field. (Will consider six years related education and experience combined of which two years must include related post high school education)
  • Required Experience:

    2 years. Working with patients who are facing multiple barriers in relation to access, engagement and retention in care. (Will consider six years related education and experience combined of which two years must include related post high school education)

    Microsoft Excel, Microsoft Word, Microsoft Outlook
  • Required Additional

    Knowledge, Skills and Abilities:

    1) Effective communication and interpersonal skills.
    2) Ability to work effectively with other team members including doctors, nursing and other clinic staff.
    3) Solid organizational skills with ability to be flexible and change rapidly as situation demands.
    4) Attention to detail and accuracy.
    5) Knowledge of current Ryan White Part B, C & D programming eligibility and procedures.
    6) Computer literacy and proficiency.
Preferred Qualifications
  • Preferred Education:

    Bachelor s degree or equivalent. If any degree/training is preferred, please specify the type.
  • Preferred Experience:

    Working with HIV
    -specific patients who are facing multiple barriers in relation to access, engagement and retention in care
  • Preferred License:
    Social work or other human services related
  • Preferred Computer Applications: NEHI, Nesiis, Iris, NE State Medicaid system
  • Preferred Additional
