Work Schedule: Monday through Friday, hours between 8am - 5pm
- Registered Nurses (RNs) will monitor, record, and report changes in the conditions of the patients and be involved in every aspect of the patients health.
- Maintain precision and accuracy in detailed reports to relay information to doctors or patients.
- Prepare, assist, and instruct patients for examinations and treatment; inform family members on health care education topics.
- Essential Functions – Joint (Floor and Phone):
- Medical Home
- Document all lab procedures and appointments on tracking forms
- Complete tracking forms and follow up on missed appointments and procedures
- Assist Health Coaches by working patient lists and scheduling appointments as directed
- Provide patient and family education to ensure adequate knowledge/awareness and recommend appropriate community resources as needed
- Educate/instruct patients according to provider direction ~98% of the time
- Perform telephone triage to determine patient needs by recording and reporting patient condition to provider
- Receive and return patient calls within 90 minutes based on medical priority ~95% of the time
- Accurately record and report patient s condition according to department policy ~98% of the time
- Maintain work areas to ensure healthy environmental conditions by cleaning and organizing equipment and rooms
- Maintain clean and organized work area according to department/provider protocol ~99% of the time
- Handle prescription renewals/inquiries to facilitate patient care; discuss questions with provider and relay information to pharmacy
- Handle prescription refills according to department/provider protocols ~99% of the time
- Provide appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the “Age Specific Criteria”
- Floor:
- Prepare exam rooms and equipment to facilitate treatment and examination of patients
- Prepare exam rooms and medical equipment according to provider protocol ~95% of the time
- Prepare patient for provider exam by reviewing medical records, weighing patient, checking vital signs, performing medication reconciliation
- Assist providers to facilitate patient treatment by retrieving tests and medical records and obtaining lab work as directed
- Dispense medications at the direction of a provider; educate patients and family members on medication usage
- Assist and/or perform phlebotomy with proper labeling and handling of specimens
- Ensure accuracy of order entry and complete documentation without error ~98% of the time
- Phone:
- Answering and routing phone calls according to protocol (≥98% success)
- Returning messages according to protocol (≥98%)
- Providing test results according to protocol (≥98%)
- Pharmacy refills processing (100%)
- Schedule and general Job Description alignment with schedule
- Graduate of an accredited school of nursing required
- If the specialty is not listed, no prior experience is required
- South Family Practice:
Minimum 1 year clinical experience and bilingual ability
- Required:
- Current American Heart Association or American Red Cross BLS (strongly preferred at time of hire, required within 3 months)
- Current valid RN License, valid compact multistate license, or a temporary permit while awaiting licensure
- Preferred:
- ACLS
