Summary Description Of Work

This position works within the Nebraska Community Support Program, providing recovery-focused targeted case management and support services to adults with severe and persistent mental illness (SPMI), including individuals with co-occurring mental health and substance use conditions. Services are delivered primarily in home and community settings, with office-based services provided when clinically appropriate.

The Community Support Specialist partners with individuals to support self-identified recovery goals, promote autonomy, wellness, and community participation, and reduce barriers to stability. The role emphasizes coordination, advocacy, engagement, and empowerment across behavioral health, substance use, and community support systems, requiring sound judgment, independent community-based work, and strong interpersonal skills.

Compensation

Wage is determined upon total years of relevant experience.

37.5 Hours Per Week

Minimum Qualifications
  • Bachelor’s degree in human services field or two years of coursework in a human services field and/or two years of experience/training and/or two years of lived recovery experience with demonstrated competencies working with individuals living with behavioral health conditions.
  • Three years’ experience working with adults living with mental illness.
  • Valid Driver’s License and acceptable driving record.
  • Ability to travel regularly within the community.
Preferred Qualifications
  • Experience in case management.
  • Experience working with area homeless service providers.
  • Proficiency with MS Word & Excel.
Essential Duties And Responsibilities
  • Establish and maintain consistent rapport with clients to provide a supportive, trauma-informed, person-centered relationship to improve client stability and functionality.
  • Engage individuals with high service needs, including those who may be difficult to reach or disengaged from services.
  • Completes strengths-based assessments and supports the development of individualized treatment and recovery goals.
  • Develops individualized treatment plans within required time frames; provides ongoing monitoring, evaluation, and updates at least every 90 days.
  • Coordinate referrals and support clients in identifying, accessing, and utilizing natural supports and formal services that align with their recovery goals, including healthcare, behavioral health, housing, education, employment, transportation, and social supports.
  • Monitor overall functioning and wellness to identify emerging needs, barriers, or risks, and coordinate timely supports to prevent crisis, relapse, or hospitalization where possible.
  • Assist clients in developing and updating personalized crisis and relapse prevention plans that reflect their preferences, strengths, and early warning signs.
  • Encourage engagement in recommended mental health and substance use treatment by reducing practical barriers, supporting appointment coordination, and promoting informed choice.
  • Communicates and collaborates routinely with treatment team members, providers, family members, natural supports, and other service systems to promote stability and community tenure.
  • Provides transition support following hospitalization or residential levels of care, in coordination with the treatment team.
  • Maintains comprehensive, accurate, and timely documentation consistent with Medicaid, DHHS, and HFS standards.
  • Provides services in diverse community settings, including homes, shelters, and other non-traditional environments, following safety and security protocols.
  • Demonstrate working knowledge of community resources and Human and Family Services (HFS) programs relevant to recovery and community integration.
