Behavioral Health Social Worker

Perform intake assessments on patients, coordinate care plans, and facilitate discharge planning within the Residential Treatment Center (RTC).
Monday through Friday; 8 AM to 5 PM

Major Responsibilities & Duties
  • Partner with intake team members to perform intake assessments, assist with collecting consent for treatment, release of information, and provide program orientation.
  • Facilitate care for the patient through referral to appropriate resources in the community or within the Boys Town system.
  • Provide leadership in the identification and acquisition of resources (local, state, and federal) for patients and their families.
  • Participate in the multi-disciplinary RTC team by assisting with data collection, analysis, and reporting, answering referral calls/questions, coordinating activities and follow‑up steps, and acting as a resource for physicians and therapists.
  • Assume the duties of the discharge planning coordinator and provide oversight of the discharge planning process. Review active patient discharge plans daily as needed.
  • Reassess the discharge planning process on a regular basis (at least quarterly), review discharge plans in closed medical record review to determine staff responsiveness to the patient’s post‑discharge needs, and review re‑admissions for trends.
  • Coordinate activities and follow‑up to meet identified needs of mental health patients and their families.
  • Maintain patient records, prepare complete, accurate, and timely reports, and monitor progress toward treatment goals.
  • Comply with all applicable standards, statutes, rules, regulations, policies, and procedures.
  • Maintain working knowledge of relevant medical/legal issues that impact patient care.
  • Participate in quality improvement activities through staff meetings, committees, continuing education, and professional activities.
Knowledge, Skills, and Abilities
  • Ability to utilize a variety of computer programs and applications to manage work, including familiarity with Windows and Office programs.
  • Ability to communicate concisely, orally and in writing, with patients, families, and staff and to collaborate with the health‑care team and community agencies to enhance patient care.
  • Knowledge of community resources.
  • Ability to work with diverse patient groups and their families.
Required Qualifications
  • Bachelor’s degree from an accredited school of social work.
  • Certification, Licensure, or Provisional Licensure as a Social Worker in the State of NE.
  • Minimum 21 years of age.
Preferred Qualifications
  • Minimum of two years of experience working with the social services needs of the child and adolescent mentally ill population.
  • Minimum of two years’ experience in utilization review and/or other healthcare‑related practices.
  • Other duties may be required and assigned as determined by a supervisor.
  • Serves as a role model in carrying out activities and behaviors that reflect the values and principles of the Boys Town mission.
Physical Requirements , Equipment Usage, Work Environment
  • Light to medium work, occasional squatting, kneeling, negotiating to the ground into a quadruped position, crawling, lying on the ground while maintaining a variable static force to restrain a child or simulate restraining, and occasional forceful exertion of the upper extremities. Tasks may require functional use of bilateral lower extremities and upper extremities for up to 3 minutes with forces up to 50 lbs of static force.

    Walking and standing vary throughout the day and may be affected by outside environmental conditions. The environment includes constant interruptions, inappropriate behaviors, and potential verbal and physical threats from youth.
