Overview

Purpose and scope:
Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse, in accordance with organization policies and applicable regulations. Responsible for the setup and operation of hemodialysis machines, and for maintaining a safe and clean working environment. Supports the organization’s Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction, and participates in process improvement activities to help patients achieve QEP goals.

Responsibilities
  • Patient Related —

    Education:

    Provide patient-specific education regarding adequacy measures where appropriate (Online Clearance Monitoring, Adequacy Monitoring Program, Urea Kinetic Modeling) and disease process/access.
  • Treatment: Welcome assigned patients and check on their wellbeing since their last treatment.
  • Evaluate vascular access for patency, perform cannulation, and administer heparin as delegated or allowed by state law.
  • Obtain pre- and post-treatment vital signs and weight; perform pre-treatment vascular access evaluation.
  • Initiate dialysis treatment per orders (QB and QD).
  • Evaluate intradialytic problems and provide interventions as prescribed by physician order or as directed.
  • Monitor patients’ response to dialysis therapy.
  • Obtain vital signs prior to reinfusion; discontinue treatment per procedures and evaluate patient prior to termination of venous access.
  • Obtain hemostasis and apply dressings; evaluate patient prior to discharge.
  • Document pre- and post-dialysis evaluation, weight, and vital signs with initial identification.
  • Calculate and enter machine programming for UF goal, treatment time, and UF modeling as prescribed; ensure safety alarms are enabled (NVL 160).
  • Record accurate treatment data on the Hemodialysis Treatment Sheet with initial identification; document progress notes as applicable.
  • Report any significant changes in patient condition to the registered nurse or supervisor; observe patient and perform machine safety checks per facility policy.
  • Intervene for unusual patient status and document response; monitor vascular access pressures and intervene as needed, reporting findings.
  • Obtain prescribed laboratory tests and prepare specimens; ensure blood spills are cleaned per policy.
  • Prepare, organize, and utilize supplies and equipment to prevent wastage; monitor patients performing self-care under RN supervision.
  • Staff Related: Assist in training direct patient care staff under supervision of a nurse.
  • Education/Communication: Follow organization policies and procedures, maintain a safe and clean environment, disinfect equipment between treatments, prepare for treatment, test pretreatment alarms (including PHT), and initiate SDS system.
  • Record Keeping: Participate in CQI activities; enter data accurately; review treatment sheets for completeness and nursing signatures; ensure missing entries are completed; prepare patient charts and lab requisitions, collect and process lab samples, and coordinate with lab courier.
  • Infection Control: Collect information for infection control audits; support adherence to infection control practices; follow policies and educate patients and families as allowed by law.
  • Transitional Care Unit: Manage care for new patients through their first 4 weeks, reinforce education, coordinate with other disciplines, and assist with transition to home dialysis options and supply/inventory needs.

  • Education and Licenses: High School diploma or GED required; CMS-approved certification within the required state or CMS timeline; licensure and training as required; demonstrated commitment to organization values.

    Required Skills:

    Prior patient care experience in a hospital setting or related facility preferred; continued employment is dependent on successful dialysis training program and CPR certification.
