Summary of WorkThis position assists InSHAPE® members to develop and implement their personal wellness program, providing support, education, and motivation related to physical fitness, nutrition, and wellness. The mentor encourages by example, trains volunteer and other mentors, instructs individuals and groups, works independently in community settings, and collects data on participant progress.
Compensation: $41,300 to $52,240 per year (salary is based on total years of relevant experience)
Work Schedule:
40 hours per week
- Personal Trainer Certification
- Current CPR and First Aid Certifications
- Bachelor’s Degree in Health Science, Social Work, Rehabilitation, Psychology, Health Promotion or related field preferred
- Ability to perform exercises and movements as demonstrations for clients
- Physically active, identifies and believes in the holistic benefits of good health
- A commitment to decreasing health disparity and reducing social stigma for individuals with mental illness
- Provide community-based support services to ensure implementation of client’s health goals
- Manage a caseload of approximately 20–25 clients to establish and implement individual InSHAPE® plans
- Collaborate with clients, program staff, and the InSHAPE® Coordinator to design and support individual member plans
- Deliver fitness training, monitoring, and continuing reassessment of exercise, nutrition and healthcare commitments
- Advise participants about safety concerns and demonstrate exercises or maneuvers as needed
- Provide clients with safe, reasonable exercises they can perform in the gym as well as at home or in the community
- Facilitate peer relationships among participants
- Participate in activities with members when support is needed
- Help members maintain focus on personal goals
- Encourage member participation and movement along the continuum of wellness‑related lifestyle changes
- Administer fitness assessments as needed and record data
- Share member participation and progress with the InSHAPE® Coordinator to assist in documentation for project outcomes
- Track and document member participation in activities
- Work with clients, volunteers and community partners to increase client independence
- Help plan and facilitate quarterly celebration and recognition events for participants
- Assist in facilitation of team building activities
- Provide instruction and facilitation for group activities
- Assist in training new mentors, interns and volunteer mentors
- Supervise volunteer mentors
- Display a courteous and caring attitude at all times toward clients, volunteers and visitors of Heartland Family Service
- Cooperate and collaborate with program area staff, volunteers, and other agency staff
- Abide by all specific program and agency procedures, policies and requirements
- Develop personal and program‑related skills through participation in internal and external training opportunities
- Create, maintain and share a dynamic self‑care plan as appropriate
- Make connections between the agency and the larger community to contribute to fundraising and friend‑raising efforts
- Perform other program‑related duties as assigned
