Responsibilities

In this role, you’ll own key responsibilities that demand both empathy and strategic thinking. Conduct comprehensive patient assessments to determine risk, need, and potential barriers to optimal outcomes. Develop and monitor robust, evidence-based care plans in collaboration with physicians, social workers, case managers, and families. Lead discharge planning discussions, coordinate post-acute services, and secure community resources that ensure smooth transitions and ongoing adherence.

Perform utilization reviews, document outcomes, and advocate for patient needs within payer and system processes. Manage complex cases with accountability for timely interventions, risk mitigation, and measurable improvements in readmission rates and patient satisfaction. Compensation & Benefits

Competitive benefits are a cornerstone of this assignment. Expect a weekly pay range of $2,208-$2,327, with a contract that starts on 02/23/2026 for several weeks, and extensions available based on performance, patient needs, and organizational priorities.
