Position Summary

The registered nurse is accountable for the delivery and supervision of safe, quality care that is individualized for their assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation). The RN is responsible for the assessment, treatment and care of patients in populations and ages as assigned. The RN accepts and promotes professional development as an integral part of nursing practice.

The RN will assess, analyze, customize, coordinate and communicate the patient s plan of care and activities collaboratively with other members of the health care team. Families and/or significant others will be included within the plan of care as appropriate. The RN will direct, delegate and manage the care delivered by colleagues and subordinates. The RN performs all duties in a manner that respects and supports the family-centered care model.

Care will reflect the philosophy of the Division of Nursing, the Mission and Core Values of the Sisters of Providence, and National Patient Safety Standards.

Position Specific Responsibilities
  • Possesses knowledge of developmental needs and is competent in the assessment and treatment as it relates to the age of the patient population served.
  • Conducts and documents a comprehensive patient assessment upon admission.
  • Continually reassesses and applies appropriate advocacy and interventions as indicated.
  • Interprets a patient s self-assessment and behavior with understanding of cognitive, physical, emotional, psychosocial and maturation factors.
  • Actively involves patient and family in the collaborative plan of care.
  • Promotes safe patient care practices: e.g., hand washing, patient identification, infection risk reduction, medication safety and reconciliation, falls prevention, and patient/family concerns and access to the healthcare team.
  • Collaborates with patient, family and healthcare team to develop, document, implement and monitor an individualized plan of care based on best practice and evidence-based medicine.
  • Provides, documents, and consults to assure patient and family education in support of the treatment plan.
  • Adaptive to change and updated patient care practices within an evolving healthcare environment.
  • Collaborates with team members, patient, family and others to develop a proactive discharge plan.
  • Performs procedures and treatments according to department protocols, guidelines, standards and the Nurse Practice Act.
  • Legibly and electronically documents patient care activities accurately and timely.
  • Competent in input and retrieval of information in written and electronic documentation.
  • Delivers safe, quality care in an efficient, cost-effective manner.
  • Supports orientation of new staff and students.
  • Acquires training to perform preceptor and charge nurse functions when assigned.
  • Demonstrates advancement of professional nursing practice through continuing education, required training, certification, and department participation.
  • Ensures competencies, certifications, safety information and courses are completed as required.
  • Identifies concerns and involves colleagues and management to promote a safe patient care environment.
  • Engages in positive communication and ongoing development of self and team.
  • Promotes an environment that fosters open communication and patient- and family-centered care.
  • Communicates adaptively based on patient/family needs.
  • Attends or reviews minutes for scheduled staff meetings (minimum 50% attendance) and stays informed on meeting content.
  • Models collaborative practice and relationships with other health care professionals.
  • Recognizes the impact of cultural diversity on relationships.
  • Rnds with physicians and team to promote continuity of care and patient confidence.
  • Monitors patient condition, notes changes, and takes appropriate action.
  • Communicates patient status using SBAR and handoffs; utilizes quality/safety tools in communication.
