Job Summary:
  • Oversees appropriate patient and personnel assignments
  • Coordinates patient care and referrals
  • Ensures patient needs are continually assessed
  • Develops, implements, and updates individualized care plans
  • Provides the Executive Director with data on patients and health services
  • Develops, evaluates, reviews, revises, and implements policies, procedures, goals, and standards of care related to patient services and staff functions
  • Adjusts staffing levels as needed
  • Conducts meetings and reviews patient care reports with staff
  • Supports a culture of safety and quality practices that do not harm patients or interfere with treatment
  • Coordinates patient services by reviewing treatment plans, conducting case conferences, reviewing clinical notes and patient evaluations, and determining staffing needs
  • Participates in hiring, training, evaluating, and disciplining staff within policy to retain quality staff
  • Schedules staff to ensure proper coverage within authorized levels
  • Coordinates staff orientation and ongoing training
  • Performs performance evaluations in a timely, measurable, and objective manner
  • Communicates performance and staffing information to the Administrator
  • Maintains knowledge of current Medicare, Medicaid, and third-party payer requirements for home care reimbursement
  • Communicates with physicians, payers, patients/families, outpatient and inpatient services regarding patient care and status
  • Stays updated on new methods, products, and trends, and shares this information with staff
  • Participates in the agency’s Quality Assurance and Performance Improvement (QAPI) programs
  • Understands applicable state and federal regulations related to home health and communicates updates to staff
  • Understands the OASIS data collection and reporting process
  • Provides direct patient care as needed
