We are seeking an experienced Adults Social Worker to join a busy Adult Social Care team in Oldham. This is an excellent opportunity to work within a forward-thinking local authority, supporting adults to achieve positive outcomes through strengths-based and person-centred practice.

You will manage a varied caseload and work with adults with complex needs, promoting independence while ensuring statutory duties are met.

• Undertake Care Act assessments, reviews, and support planning.
• Complete safeguarding enquiries and risk assessments.
• Carry out Mental Capacity Assessments and Best Interest decisions.
• Support hospital discharge and community-based interventions where required.
• Work collaboratively with health professionals, families, and partner agencies.
• Promote independence, wellbeing, and positive risk-taking.
• Maintain accurate, timely, and high-quality case recording.
