• Support young adults transitioning from care into independent living
• Undertake statutory assessments in line with legislation and local authority procedures
• Develop, implement, and review person-centred transition and care plans
• Work closely with families, carers, and multi-agency partners to ensure coordinated support
• Monitor outcomes and adapt plans to meet changing needs
• Advocate on behalf of young adults to secure appropriate services, resources, and opportunities
• Maintain accurate, timely, and compliant case records
