We are looking for a motivated Clinical Pharmacist to join our friendly and supportive GP practice team. Main duties of the job

The Clinical Pharmacist will support the safe and effective use of medicines within the GP practice. Key duties include carrying out structured medication reviews, supporting the management of long-term conditions, and providing expert medicines advice to patients and the wider practice team. The role also involves supporting prescribing processes, improving medicines safety, and contributing to quality improvement and service development within primary care.

Role Purpose

The Clinical Pharmacist will work as part of the GP practice team to deliver high-quality, patient-centred pharmaceutical care. The role focuses on medicines optimisation, improving patient safety, supporting long-term condition management, and reducing medicines-related workload for GPs.

Key Responsibilities

  • Conduct structured medication reviews, including for patients with complex or long-term conditions
  • Support safe, effective, and cost-effective prescribing in line with local and national guidance
  • Provide expert medicines advice to patients, GPs, nurses, and the wider practice team
  • Manage and resolve medicines-related queries, including repeat prescribing and reconciliation
  • Support chronic disease management such as hypertension, diabetes, asthma, and cardiovascular disease
  • Identify and manage medicines safety issues, including high-risk medicines and monitoring requirements
  • Support prescribing audits, quality improvement initiatives, and practice protocols
  • Contribute to service development and improved use of clinical systems
  • Maintain accurate clinical records and comply with information governance requirements

Professional Responsibilities

  • Work within professional, ethical, and legal frameworks
  • Maintain professional registration and continuing professional development (CPD)
  • Participate in supervision, training, and appraisal as required
  • Work collaboratively within the practice and PCN team

Working Relationships

  • GPs and GP registrars
  • Practice nurses and healthcare assistants
  • Other PCN staff including pharmacists and pharmacy technicians
  • Community pharmacies and secondary care teams

Person Specification Qualifications
  • Master of Pharmacy (MPharm) degree or equivalent
  • Registered with the General Pharmaceutical Council (GPhC)
  • Evidence of continuing professional development (CPD)
  • Experience working as a registered pharmacist
  • Experience of medicines optimisation and medication reviews
  • Experience of working collaboratively within a healthcare team
  • Experience of providing patient-centred care and clinical advice
  • Independent Prescriber (or working towards qualification)
  • Primary Care Pharmacy Education Pathway (PCPEP) or equivalent training
