Reablement Support Worker (Community Reablement)
Role Overview: Mio Care’s Community Reablement Service offers planned, short-term intensive assistance to people in their own home, helping them to regain the skills needed to live as independently as possible and increase confidence in their own abilities.
As a Reablement Support Worker you’ll provide support to customers (and their families and carers), completing tasks that will promote and maintain their physical, mental and emotional health, enabling them to remain in their own home.
You’ll work as an effective member of the team that provides high‑quality service seven days a week, promoting independence while meeting the requirements of the Care Quality Commission (CQC), and Mio Care’s departmental policies, procedures and guidelines.
Key Responsibilities- Assist with the monitoring, implementation and evaluation of care plans and reablement programmes.
- Complete written records as required with people we support, their carer(s) and family, following discussions regarding agreed goal planning.
- Provide flexible, personal and practical support (inclusive of personal care needs) which is respectful of people’s privacy and dignity and in line with their assessed need.
- Maintain accurate records, contribute to reviews/assessments and team meetings and liaise with other agencies and professionals such as social workers, nurses and therapists when required.
