Aberdeen Village Registered Nurse (RN) Case Manager

Aberdeen Village is a faith-based, not-for-profit organization dedicated to providing high-quality care for residents in a supportive community environment. We are looking for a Registered Nurse (RN) Case Manager to supervise Medicare Part A and manage the care of residents during Medicare stays.

Starting at $40.00 an hour plus shift and weekend differential with our premium pay plan, with additional pay based upon experience.

• Supervise Medicare Part A and facilitate managed care programs by determining eligibility for Medicare.
• Ensure all required paperwork is completed during residents’ Medicare stays.
• Monitor and coordinate care while residents are in Medicare stays.
• Follow federal and state guidelines and regulations and other required guidelines.
• Create an environment that values residents first and models person-centered care.

• Must possess a nursing degree from an accredited college or university or be a graduate of an approved RN program.
• Must possess a current active license to practice as an RN in this state.
• Knowledge of nursing and medical practices and procedures as well as laws, regulations and guidelines governing long term care.
