Case Manager/Social Worker – Recover-Care Healthcare

Meadowbrook Rehabilitation Hospital, located in Gardner, KS, is a specialty rehabilitation hospital that provides acute inpatient rehab and brain injury rehabilitation. We are looking for a case manager to join our team.

Summary

The case manager is the resource person in the area of inpatient case management and discharge planning.

Responsibilities
  • Conduct a psychosocial history to determine pre-injury issues that need to be addressed.
  • Establish and carry out an individualized treatment plan upon completion of psychosocial history, if applicable.
  • Provide counseling to clients, families, and/or significant others.
  • Develop and implement family and children support groups as needed.
  • Schedule, arrange, and moderate the program for family education groups.
  • Participate in team rounds and family conferences as scheduled through the case manager.
  • Maintain records and prepare reports according to facility procedures.
  • Participate in and document internal and external professional educational activities to enhance personal and professional growth.
  • Perform public relations tasks as directed by supervisor.
  • Coordinate the client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialists, and other disciplines as needed.
  • Serve as an integral part of the interdisciplinary team.
  • Key component in assisting in patient discharge planning.
  • Conduct post-discharge follow-up as needed.
  • Perform other tasks as assigned by the immediate supervisor.
Qualifications
  • Bachelor's in Social Work or related field (Preferred).
  • Formal Education/Formal Training:
    Degree/license in healthcare (Preferred).
  • Minimum of two years’ experience in a hospital/health care setting, preferably in an Inpatient Rehab Facility (IRF).
