Midland Care Connection is a not‑for‑profit, community‑based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All‑Inclusive Care for the Elderly) program. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. A PACE Support Specialist at Midland Care helps keep the PACE program running smoothly by coordinating schedules, appointments, and transportation for participants, maintaining accurate electronic records, supporting the interdisciplinary care team, assisting with home medical equipment orders, and helping with quality improvement activities. The role ensures that daily operations flow efficiently so participants receive timely, well‑organized care.

• Experience with scheduling, clerical tasks, or administrative support in a healthcare or office setting
• Strong organizational, multitasking, and time‑management skills
• Good verbal and written communication skills
• Basic proficiency with computers, including data entry and electronic documentation systems
• Ability to work collaboratively with interdisciplinary care teams
• Attention to detail and ability to maintain accurate records

• Prior experience in healthcare, social services, or long‑term care settings
• Familiarity with PACE programs or eldercare service coordination
• Experience with electronic health records (EHR) and scheduling systems
• Comfort supporting clinical and operational needs in a fast‑paced environment
