The Licensed Practical Nurse (LPN) provides general nursing care to patients, families, and caregivers in the Medical Group practices. The LPN assists and advocates for the patient's health in support of patient-centered care, collaborating with physicians, registered nurses, and multidisciplinary team members. Tasks include case finding, health coaching, and supportive care under supervision, in accordance with NYS regulations and standards.

• Gather patient measurements and data
• Record and report patient findings
• Prepare patients for examinations and treatments
• Assist with examinations and treatments
• Support supportive care tasks under clinical direction
• Promote self-care and management plans
• Maintain safety protocols and equipment
• Ensure a clean, safe work environment
• Participate in ongoing education and quality improvement
• Communicate effectively within the healthcare team
• Respond to patient requests and communicate with providers

Required:

• Graduate of an accredited LPN program

Preferred:

• Bachelor's or Master's in Nursing from an accredited program

Licensure/Certification:

• Current NYS LPN license
• Basic Life Support (BLS) certification within 6 weeks of hire

Pay range: $18.00/hr - $29.00/hr full-time, day shift, 8.5 hours per day.
