- Active, unencumbered LPN/LVN license in the state of practice
- Compact licenses transferred to state of residence within 90 days
- Current CPR certification (hands‑on component) – training provided post‑hire if Clave required

- Six months prior hands‑on nursing experience preferred but not required
- Reliable transportation
- One‑to‑one patient care in the home environment
- Execute physician‑prescribed Plan of Care and maintain compliant documentation in the system of record
- Administer prescribed medication, treatments, and therapies
- Conduct patient assessments and coordinate care among the care team
- Promote health, provide teaching, and train family members in patient care
