SUMMARYAs a Licensed Practical Nurse, you will provide professional patient care to long‑term and post‑acute care residents, direct and monitor the care provided by CNAs, and ensure accurate documentation and recording.
RESPONSIBILITIES- Timely, accurate medication administration of assigned kåtcare team and concurrent required documentation.
- Perform treatments and document progress as per physician orders and facility protocol.
- Ensure concise, pertinent and complete resident care documentation using computerized medical record processes and initiate appropriate care pathway tools.
- Receive and give relevant reports, written and verbal, to appropriate staff per departmental protocols.
- Monitor residents and report relevant condition status to charge nurse/supervisor and/or physicians in a timely manner.
- Complete pre‑reviews of medication administration records as per facility protocols.
- Complete required MDS documentation and all related observations on time.
- Support the CNA team through direct ADL care of residents.
- Graduate from an accredited School of Practical Nursing.
- Current licensure in the State of New York.
- One year of experience preferred.
- BLS – Basic Life Support, American Heart Association (AHA)
- LPN – Licensed Practical Nurse, New York State Education Department (NYSED)
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing or squatting.
