• Provide one-to-one patient care in a home environment.
• Execute physician-prescribed plan of care and maintain compliant documentation in the system of record.
• Administer prescribed medications, treatments, and therapies.
• Conduct patient assessments and coordinate care.
• Promote health, teach, and train family members.
• Assist with feeding tube management and medication administration for a teenager patient with low acuity.
• Maintain communication with the local care team and provide updates as necessary.

• Active, unencumbered LPN/LVN license in the state where the clinician will practice.
• Compact licenses transferred to state of residence within 90 days.
• Current CPR certification with hands‑on component (may be obtained after hire if needed).
• TB skin test current within last 12 months.
• Six months of hands‑on nursing experience preferred but not required.
• Reliable transportation.
• Good standing license in the state of practice.
• Willingness to provide in-home nursing care to infants, children, and adolescents.
