- Provide one-to-one patient care in a home environment.
- Execute physician-prescribed plan of care and maintain compliant documentation in the system of record.
- Administer prescribed medications, treatments, and therapies.
- Conduct patient assessments and coordinate care.
- Promote health, teach, and train family members.
- Assist with feeding tube management and medication administration for a teenager patient with low acuity.
- Maintain communication with the local care team and provide updates as necessary.
- Active, unencumbered LPN/LVN license in the state where the clinician will practice.
- Compact licenses transferred to state of residence within 90 days.
- Current CPR certification with hands‑on component (may be obtained after hire if needed).
- TB skin test current within last 12 months.
- Six months of hands‑on nursing experience preferred but not required.
- Reliable transportation.
- Good standing license in the state of practice.
- Willingness to provide in-home nursing care to infants, children, and adolescents.
