• Sunday through Saturday (8 AM–4 PM) & (10 PM–6 AM)

• Chouteau, OK

• Infant

• High Acuity (Trach & Vent)

• One-to-one patient care in a home environment
• Execution of physician-prescribed plan of care and compliant documentation in the system of record
• Administration of prescribed medication, treatments, and therapies
• Patient assessments and coordination of care
• Health promotion, teaching, and training of family members

• Active, unencumbered LPN/LVN license in the state of practice
• Compact licenses must be transferred within 90 days
• Current CPR certification (hands‑on component)
• TB skin test current within last 12 months
• Six months prior hands‑on nursing experience preferred but not required
• Reliable transportation
