Advanced Clinical Practitioner - Acute Care Team
At East Lancashire Hospitals NHS Trust we strive to provide a first class Safe, Personal and Effective service to all our patients.
We are seeking to appoint a qualified Advanced Clinical Practitioner (ACP) from a Registered Nurse or Allied Health Professional background, to join our Acute Care Team. Our multidisciplinary team comprises of a Nurse Consultant, Advanced Clinical Practitioners, Acute Care practitioners, health care assistants and vascular access services (including phlebotomy).
The ACP role on the Acute Care Team is a dynamic and multi-faceted position that encompasses all pillars of advanced practice. The job involves the management of unwell patients across all ELHT sites, developing plans and offering advice to patients suffering from, or at risk of critical illness. The role also supports clinical and site risk response supportive of operational pressures and patient safety.
Interested applicants must be a qualified ACP and possess a MSc in Advanced Clinical Practice (or equivalent). The ability to work as an independent Non-Medical Prescriber is essential therefore applicants must also possess a NMP qualification.

Main duties of the jobThe Advanced Clinical Practitioner will provide highly specialised, evidence-based care across ELHT, to those patients with chronic and acute conditions using advanced clinical knowledge
Work closely with the Intensive Care Nursing and Medical Teams to facilitate early and timely admission to critical care and safe discharge from critical care
To provide the infrastructure that addresses the knowledge and skills deficits of ward teams in delivering acute care skills to patients.
Support the Implementation of national guidelines or evidence-based practice to improve patient care. Addresses clinical governance issues promptly.
Provide leadership support for the Acute Care Team.
Provide clinical management of patients
Support clinical site management and flow

Job responsibilitiesTo provide advice, support and direction to ensure holistic patient care for acutely ill and deteriorating patients across ELHT in conjunction with the multi disciplinary team over the 24 hour period.

The Advanced Clinical Practitioner will function independently or within a clinical team and will demonstrate competence complementary to the MDT role in the delivery of health care.
Provide expert advice and to ensure the needs of the patient are met by leading, challenging and changing practice within the clinical setting.
To accept new and follow-up referrals
To discharge patients from caseload as appropriate
To undertake clinical audit and research
Recognises and acts as an advocate for patients, carers, service and organisation.
Lead change as required and ensure resolution of conflict.
Undertake and lead quality improvement projects to support service development.
Collaborate with the resuscitation team to develop and deliver education to improve cardiac arrest outcomes
Person Specification training and qualifications- First level registered Nurse/Allied Health Professional currently working at an Advanced Practitioner level
- Non Medical prescribing module
- MSc in Advanced Practice or equivalent
- Advanced Clinical Skills
- Recognised advanced specialist qualification in acute care
- NMC Approved mentor preparation course/ or equivalent.
- ALS provider
- Teaching Qualification
- Experience in clinical supervision
- ALS Instructor
- Transfer Training (e.g. SCITT)
- Professional registration with NMC/HCPC Professional Body
- 5 years post-registration experience (or equivalent)
