Description

Applications are now open for a Licensed Clinical Social Worker, serving as a Behavioral Health Specialist for Pushmataha Family Medical Center, at our Clayton, OK location. We have recently incorporated behavioral health services into our primary care service areas. These services aim to connect individuals to mental healthcare, who may traditionally be unable to access it. The LCSW will work collaboratively, with medical staff, provide consultation, and direct care for a variety of presenting mental health needs.

In alignment with an integrated approach, community partnerships will be strengthened by a more efficient systems of referrals for those with ongoing mental health needs.

Requirements

The Licensed Clinical Social Worker (LCSW) will participate in the implementation of Integrated Behavioral Health services at Pushmataha Family Medical Center (PFMC).

PFMC has incorporated integrated behavioral health services in our primary care settings as it provides a service model which assists in reaching patients who commonly have limited access to traditional mental health treatment.

The LCSW will work collaboratively, as a Behavioral Health Specialist, with medical staff to implement a system of comprehensive screenings, consultation with the inter-disciplinary team, and provision of direct care practices to improve overall health outcomes and strengthen access and utilization of mental healthcare for our patients.

In addition to providing evidence-based treatment for traditional mental health issues the LCSW will help identify and address relevant behaviorally driven health concerns that arise throughout their medical care.

Consistent with an integrative care model, the LCSW will work to strengthen collaborative relationships with other service providers in the community to promote a more effective system of referrals for patients with ongoing mental health needs.

The LCSW will occupy an essential position in helping PFMC fulfill its mission of providing affordable, comprehensive, and high-quality healthcare to all.
