Responsible for providing skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse. Must assess, plan, implement, and evaluate patients' needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

Additional Job Description

• Graduate of an accredited practical nursing program; consideration given for comparable education experience and successful licensure.
• Current LPN license in the state of practice.

• +1 year preferred experience.
• Possesses and maintains current CPR certification.

• Complies with accepted professional standards and principles.
• Understands Hospice philosophy; issues related to death/dying and grief.
• Complies with Unity Health Hospice Health Status requirements.
• Ability to provide good written documentation in a timely manner.
• Flexible and cooperative, able to work within an interdisciplinary team.
• Handles emergencies without assistance; can assist falling patients and perform CPR alone.
• Assesses home environment for safety and potential abuse.
• Familiar with driving at night/hazardous conditions; can read maps and schedule visits independently.
• Maintains confidentiality and communicates with agency/medical staff.
