• Provide care and services for a group of hospice patients across multiple settings (homes, assisted living, hospitals).
• Complete hospice admissions and patient visits as needed.
• Answer calls from patients, families, and customers.
• Make nursing visits for symptom management, deaths, and admissions.
• Work on-call after hours: seven nights on and seven nights off.

• Current RN licensure in good standing in the state(s) of practice with at least one year of experience.
• Admissions and on-call experience preferred.
• Strong teamwork, collaboration, and communication skills with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Independent work capability with strong time‑management, triage, and documentation skills.
• Ability to learn and use an EMR system.
• Valid driver’s license, reliable transportation, and auto insurance for service area travel.
