As a Clinical Liaison
, you will serve as a key connection between patients, families, referral partners, and care teams—using your clinical expertise and communication skills to ensure patients have access to the right level of rehabilitation care.
In this role, you will:
- 🤝 Build and maintain referral relationships to support appropriate admissions
- 📝 Coordinate intake and pre-admission screening processes
- 🏥 Perform on-site clinical assessments at referring facilities
- 💬 Educate patients and families on rehabilitation options and expectations
- 📊 Gather and review patient benefit and resource information
- 📞 Serve as the primary point of contact for case managers, discharge planners, payors, and hospital partners

Qualified candidates will have:
- 🎓 Clinical licensure as RN, PT, OT, SLP, or MSW
- Bachelor’s degree
- Prior clinical experience with strong assessment and EMR skills (preferred)
- Marketing or sales experience (preferred)
- Formal sales training (preferred)
- 🚗 Valid driver’s license
