When every moment matters, your experience makes the difference. Interim Health Care of Oklahoma City is seeking an experienced Hospice RN Case Manager to join our small, close-knit hospice team. This is a part‑time position focused solely on Hospice patients
, providing comfort, dignity, and peace to patients and families during one of life’s most meaningful journeys.

This position is best suited for nurses with prior hospice experience who are confident managing both the clinical and emotional aspects of end‑of‑life care.

• Manage a hospice caseload with individualized plans of care
• Provide symptom management, direct patient care, and emotional support
• Serve as a primary point of contact for patients, families, and caregivers
• Collaborate with physicians and the interdisciplinary care team
• Complete accurate documentation in compliance with Medicare, Medicaid, and agency standards
• Participate in the on‑call rotation as scheduled

• Active RN license in Oklahoma
• Hospice experience required (1+ year strongly preferred)
• Strong assessment, critical‑thinking, and organizational skills
• Compassionate, professional communication style
• Ability to work independently while remaining a strong team partner
• Valid driver’s license, auto insurance, and reliable transportation
