The Integrated Care Manager will be a member of Healthy Minds’ community initiatives team and support the Oklahoma Integrated Care Resource Center, which is funded by an Oklahoma TSET Legacy Grant. This position will be responsible for advancing integrated behavioral health best practices into routine health care and increasing the utilization of evidence-based treatment strategies for patients in primary care settings.

The Integrated Care Manager is an experienced project manager who has worked in grant implementation and has knowledge of projects in integrated behavioral health care and the Collaborative Care Model. The Integrated Care Manager has project management experience that includes coordination of activities across a variety of project teams, aligning all partners, tasks and resources to maintain momentum, support implementation efforts, and ensure adherence to grant requirements.

This position reports to the Assistant Director of Community Initiatives and is full-time, exempt.

This position is funded by a time-limited grant from the Oklahoma Tobacco Settlement Endowment Trust and subject to annual renewal through December 31, 2028.

• Coordinate day-to-day activities related to the TSET grant to ensure timely completion of goals and deliverables
• Serve as the primary liaison between internal teams, external partners, and health care providers involved in grant activities.
• Facilitate communication across clinical, administrative, and community partners to ensure alignment of project objectives.
• Build and manage relationships with sub-contractors and ensure compliance with key deadlines and obligations.
• Establish relationships with community partners participating in grant activities on behalf of Healthy Minds.
• Organize and support interdisciplinary meetings, work groups, and trainings as needed related to grant activities.
• Support project evaluation efforts by maintaining key records, project files, and communication accordingly.
• Promote awareness and invite support of Healthy Minds’ integrated behavioral health initiatives.
• Maintain professional competencies in subject matter through training, use of mentors/consultants, and other development activities.
• Other duties as assigned.

• Bachelor’s degree in public policy, public health, public administration, behavioral health, community health or related discipline. Master’s degree preferred.
• Four years of experience in health care or behavioral health project management and systems improvement management.
• Experience with applied subject matter expertise in behavioral health integration practices, health care workflows/care transitions, and evidence-based substance use and mental health practices.
• Knowledge of grant implementation and grant management.
• Strong written and verbal communication skills.
• Experience in a nonprofit or public service role is preferred.
• Advanced knowledge of Microsoft Office, especially Excel, PowerPoint, and Outlook.
• Experience working with software solutions for organizational communication (e.g., Microsoft Teams, Asana, Basecamp, etc.) preferred.
