Position

Intake Clerk is primarily responsible for receiving and processing all new admissions and readmissions. As the gatekeeper, individuals will generally serve as the first point of contact for facility customers to ensure that all demographic and payer information is received for timely delivery of medications. He/she will also be responsible for processing census changes.

Schedule: Monday – Friday:
Mid Shift and 2nd shift available (10:30AM - 7PM | 11:30AM - 8PM OR 3PM - 11:30PM) with rotating weekends and holidays

Key Responsibilities
  • Process all admission demographic and billing information and ensure completion before sending to other departments for medication order entry, processing and delivery. Ensure that resident demographic information is requested as needed, received timely and verified/entered accurately. Obtains any missing information within 24-48 hours and complete patient profile.
  • Triage, accurately process and forward/complete admission information from an electronic queue.
  • Ensure receipt of insurance cards/third party information and Pharmacy Enrollment Agreement. Verify data and enter accurately into system.
  • Verify and prioritize work based on operations and billing department deadlines.
  • Ensure corroborating demographic and payer information used to complete admission are downloaded, triaged and associated to resident chart.
  • Process and accurately work facility discharges.
  • Work in collaboration with appropriate departments to address any coverage issues prior to order processing. Includes, but is not limited to insurance verification, additional documentation or follow-up as needed with facility or physician.
  • Ensure that all responsible party information and contact information is verified and entered in the patient chart.
  • Processes daily census related to admissions, discharges/deaths, room changes, and complies with all department policies and procedures.
  • Establish relationships with appropriate staff in customer facilities to further ensure that timely information is received on new admissions and census changes.
  • Set up/on-board new facility customer accounts for all admission data that are brought on to Remedi service. Work with Remedi and facility account management and on-boarding team to ensure all resident information is received and entered prior to go-live date.
  • Perform all other duties as assigned.
  • The ability to adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
Qualifications
  • High school diploma or GED equivalent.
