This position may be filled as a Level 1, 2, or 3 depending on specific education, experience, and license requirements.

A Care Management Social Worker will provide essential support and guidance to patients and their families as they navigate overall wellness in the OU healthcare system. The role involves comprehensive care coordination and support services to individuals and families facing complex social and health‑related challenges. The Social Worker collaborates with clients, families, healthcare professionals, and community resources to address unique needs and improve overall well‑being.

Essential

Responsibilities
  • Assess assigned patients’ unique conditions by combining data from medical diagnoses, chart reviews, personal interviews, and care team inputs.
  • Perform psychosocial assessments and relate findings to necessary clinical staff.
  • Provide or supervise plan‑of‑care interventions, including resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy (under LMSW supervision), and other support as appropriate.
  • Collaborate with the healthcare team and involve the patient and family in developing and implementing care plans.
  • Provide education to patients and families about diseases, treatments, social or financial situations, emotional stress, and other challenges.
  • Monitor care to evaluate progress toward goals and institute appropriate interventions as needed.
  • Conduct high‑risk screening, provide crisis counseling (under LMSW supervision), and educate families about abuse or neglect; document sessions.
  • Act as liaison between patients/families and clinical staff and external agencies.
  • Advocate for patient concerns and address ethical dilemmas through appropriate channels.
  • Maintain an approachable manner, treating others fairly, respectfully, and preserving dignity.
  • Complete applicable reports based on mandated reporting requirements.
  • LMSW or LCSW with 1 year experience may precept newly hired SW Level I, BSW, or MSW students.
General Responsibilities
  • Perform other duties as assigned.
Minimum Qualifications – Level 1

Education: Master of Social Work from an accredited institution.

Experience: 0‑3 years of work experience; healthcare experience preferred.

License/Certification: Current Licensed Master Social Worker (LMSW) or under supervision for Licensed Clinical Social Worker (LCSW) or LCSW from the Oklahoma State Board;
Basic Life Support certification from the American Heart Association required for inpatient setting.

Minimum Qualifications – Level 2

Education: Master of Social Work from an accredited institution.

Experience: At least 3 years of social work experience.

License/Certification: Current LCSW from the Oklahoma State Board;
Basic Life Support certification from the American Heart Association required for inpatient setting.

Minimum Qualifications – Level 3

Education: Master of Social Work from an accredited institution.

Experience: At least 3 years of social work experience.

License/Certification: Current LCSW from the Oklahoma State Board;
Basic Life Support certification from the American Heart Association required for inpatient setting.

Knowledge, Skills, and Abilities

Knowledge of principles and practices of social and psychological casework; social and welfare resources in community and hospital; current social and economic conditions, trends, and problems. Analytical skills sufficient to interpret situations and determine appropriate course of action. Ability to explain technical data in an understandable form to patients, families, and staff. Good interpersonal skills.
