Social Worker SINDC

Overview

Duration: 13 Weeks Contract (Strong Possibility of Extension)

Shift: Days; 40 hrs a week

Pay Range: $43 to $48/hr.

Under general supervision, provide comprehensive social services to patients and their families within a hospital setting.

• Deliver social services for assigned medical units
• Conduct psychosocial assessments and discharge planning
• Provide counseling related to illness, treatment, emotional stress, and social/financial challenges
• Manage high-risk cases and provide crisis intervention (abuse, neglect, end-of-life care)
• Act as a liaison between patients, families, hospital staff, and community agencies
• Support post-acute care placement and community referrals
• Advocate for patients in informed decision-making and ethical situations
• Utilize specialized communication skills when working with children
• Document interventions and outcomes accurately

• Master’s Degree in Social Work (MSW) from an accredited institution

• LMSW (strongly preferred)
• Prior hospital or acute care experience
• Strong analytical and assessment skills
• Excellent interpersonal and communication abilities
• Knowledge of community, social, and healthcare resources
