We are seeking a licensed Pharmacist to support home infusion services by ensuring safe, effective, and patient-centered pharmaceutical care. This role involves clinical review, compounding oversight, interdisciplinary collaboration, and direct patient and caregiver education to support high-quality outcomes in the home setting.
Key Responsibilities- Assess patient suitability for home infusion therapy
- Design, monitor, and evaluate individualized pharmacy care plans
- Recommend or adjust medication therapies based on clinical data and provider orders
- Prepare, compound, label, and dispense medications in compliance with USP, DEA, and state regulations
- Conduct nutritional assessments and provide education to patients and caregivers
- Collaborate with nursing and interdisciplinary teams to ensure continuity of care
- Monitor controlled substance inventory and promptly report discrepancies
- Provide accurate drug information to healthcare professionals and patients
- Participate in continuing education and professional competency programs
- Adhere to company policies, quality assurance standards, and all regulatory requirements
- Graduate of an accredited School of Pharmacy
- Active Pharmacist license in the State of Oklahoma
- One of the following:
- 1+ year of experience in sterile IV admixture procedures
- Knowledge of non-sterile compounding, immunizations, and Medication Therapy Management (MTM)
- Completion of an ASHP-accredited Pharmacy Residency Program
- Completion of an Advanced Pharmacy Practice Experience (APPE)
- Equivalent experience approved by pharmacy leadership
- Proficiency in Microsoft Word, Excel, and Power Point
- Strong customer service focus with a commitment to patient-centered care
- Experience in home infusion, home care, hospital, or clean room environments
- ASHP-accredited Pharmacy Residency completion
- 1+ year of indirect or supervisory experience
- Standard business hours with flexibility to support home infusion coordination
