Under the supervision of the Nurse Administrator, the Speech Therapist is responsible for assessing and evaluating patient care needs related to treating speech and language disorders, as well as functional training in communication, swallowing, and cognitive impairments. Based on this assessment, the Speech Therapist determines a treatment plan, implements interventions to improve and enhance well‑being, and evaluates progress.
OrganizationThis position reports to the Director of Therapy.
Essential Functions- Perform initial and ongoing assessments to evaluate current level of function.
- Administer, evaluate, and interpret tests to determine functional goals and outcomes.
- Provide written evaluations and plans of care defining therapy needs, goals, progress, and expected outcomes.
- Deliver speech therapy services in compliance with prescribed physician orders.
- Maintain plans of care by documenting client performance, modifying goals and treatment as needed, and determining discharge care plans.
- Perform discharge planning, including collaborating and educating patients and caretakers on home exercise plans.
- Coordinate services with the health care team, physicians, parents/guardians, nurses, and others.
- Document accurately according to policy, requiring minimal supervision.
- Follow all policies, procedures, licensure regulations, and state laws.
- Maintain effective communication among staff, team members, and families; serve as a resource consultant for clinical staff.
- Supervise other personnel as applicable.
- Complete OASIS assessments where allowed by state regulation.
- Perform other duties as assigned.
- Master's degree in Speech Language Pathology from an accredited program.
- Current state licensure in Oklahoma and good standing.
- Basic Cardiac Life Support (BCLS) certification.
- 1‑2 years recent home health experience preferred.
- Valid vehicle insurance, registration, and a required vehicle.
- Medical knowledge, patience, compassion, kindness, critical thinking, infection control, stamina, physical endurance, observational skills, safety knowledge, communication, leadership, and empathy.
Moderately heavy physical exertion on a regular basis such as extensive driving, assisting patients in transfer activities (wheelchair, bed, tub, commode), and providing substantial support to individuals in ambulation. Frequent use of hands, fingers, and voice; regular standing, walking, reaching, stooping, kneeling, crouching, crawling, and occasionally sitting. Occasional lifting and moving of items over 100 pounds. Vision requirements include close, color, peripheral, depth perception, and focus adjustment.

Environment includes general office, patient home settings, and automobile.
TrainingSecurity and privacy training will be provided to protect confidentiality, integrity, and availability of PCS and PCH associate and customer data.
